Requesting Your Healthcare Records

Clients may request a copy of their healthcare records maintained by this practice. Requests must be submitted in writing and should include the client’s full name, date of birth, contact information, and a description of the records requested. Requests may be submitted via email, mail, or in person. Records will be provided in accordance with Texas law and applicable timelines.

Filing a Consumer Complaint

Consumers may file a complaint regarding services received from a licensed provider with the Office of the Attorney General of Texas. Complaints can be submitted online through the Consumer Protection Division at:https://www.texasattorneygeneral.gov/consumer-protection/file-consumer-complaint
